This page explains the limits of our role and the real risks involved in pursuing U.S. medical licensure through the ITPE pathway. We'd rather you know all of this upfront than be surprised later — read it carefully, and reach out if anything is unclear.
State medical boards make all licensing decisions. We do not control board outcomes, and approval is not guaranteed.
We only accept cases we believe are viable, but some applications are still denied or delayed. Acceptance by PPG is not a prediction of board approval.
The hiring decision belongs to the employer, not to us. We present candidates we believe are strong fits for our partner institutions, but an employer may decline to extend an offer.
Acceptance into our program, completion of the roadmap, and successful credentialing are not a guarantee of placement. Timing depends on current openings at our partner institutions, and some candidates may wait longer than others for a matching opportunity.
An ITPE license from the NC Medical Board establishes your legal right to practice medicine in North Carolina. It does not, by itself, determine whether a health insurance plan will credential you into its provider network — and without network credentialing, an employer generally cannot bill for your services, regardless of your license status.
Board certification from a body such as the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA) is a common credentialing standard used by many health insurance plans. Because ABMS and AOA certification generally requires completion of a U.S. or Canadian accredited residency, most ITPE candidates will not hold it. Some insurance plans allow exceptions to this standard on a discretionary, case-by-case basis; others do not appear to.
This is a developing area, separate from anything the NC Medical Board governs. We research and share what we currently understand about credentialing pathways at the employers in our network, but we cannot guarantee network credentialing outcomes with any specific insurance plan — and this can affect a physician's practical ability to generate billable income even after a license is granted and an employer relationship is in place.
Medical board rules, interpretations, and timelines can change without notice. Requirements that apply at the start of a case may change before the process is complete.

If your qualifying employment ends for any reason — regardless of placement type — you must secure a new qualifying NC employer promptly, or the licensing process may be paused or terminated.
For rural placements specifically, on-site supervision is a legal requirement and cannot be waived. If the supervising physician withdraws or is no longer practicing on-site, that has the same effect as an employment gap and must be resolved promptly.
ITPE license holders must practice only within the scope and employer type approved by the NC Medical Board. Changes to job duties, practice location, or hours may require board approval before they take effect — and, for rural placements, so does any change to the supervising physician.
Unauthorized changes can jeopardize licensure. Neither the physician nor the employer should assume that changes are permissible without first confirming with the board.

Pursuing U.S. medical licensure through the ITPE pathway involves regulatory risk, a multi-year time commitment, and meaningful uncertainty. This pathway is not suitable for all applicants. We believe a clear-eyed understanding of these risks is a prerequisite to any engagement — not a disclosure to be buried at the bottom of a page.
