North Carolina's ITPE pathway allows international doctors to practice medicine without completing a U.S. residency. It is not a shortcut — it is a structured, four-year pathway, overseen by the NC Medical Board, that culminates in eligibility for full unrestricted licensure.

What that four-year period looks like depends on where you're placed. Physicians placed at an NC-licensed hospital practice within the hospital's own credentialing and privileging structure, the same as any other physician on staff. Physicians placed at a qualifying rural medical practice must have a designated NC-licensed physician supervising on-site — a requirement set by statute for that placement type specifically.

Hospital vs. Rural Placement

The ITPE license covers two distinct placement types, and the practice requirements differ between them:

  • Hospital placement: you practice at an NC-licensed hospital, governed by that hospital's own medical staff credentialing and privileging process — the same structure any physician on staff practices under. There is no separate, named ITPE supervising physician.
  • Rural placement: you practice at a qualifying rural medical practice, where a designated NC-licensed physician must be physically practicing on-site. That physician is named on your ITPE license and carries periodic reporting obligations to the NC Medical Board.

Most placements we coordinate are hospital-based. Which type applies to you depends on your eventual employer match.

Who the Supervising Physician Is

For rural placements specifically, the supervising physician is:

  • fully licensed in North Carolina
  • approved by the NC Medical Board
  • legally responsible for supervision-related reporting

Employment is arranged in connection with the supervision arrangement. In some cases, the supervising physician is the employer. In other cases, the supervising physician is designated by the employer. The supervising physician's stipend is typically paid by the employer institution. Hospital placements do not involve a named supervising physician or stipend arrangement of this kind.

How Rural Supervision Is Arranged

For rural placements, supervision is not automatic. It must be arranged in advance and approved by the NC Medical Board. For hospital placements, the equivalent process is standard hospital credentialing and privileging.

  • preparing your side of the required supervision documentation
  • ensuring your file is complete and ready for your employer's HR and credentialing teams
  • timing your readiness with the employer's hiring process

The supervision arrangement itself — including who your supervising physician is and confirming that arrangement meets NC Medical Board requirements — is managed by your employer's HR team, not by PPG. Our role is to bring a pool of qualified, thoroughly screened candidates to our partner employers so that once you are matched, the remaining work is largely paperwork on the employer's side. This matching step depends on current openings at our partner institutions.

The Path to Full Licensure

Documentation & Employer Offer

Before applying, you must have a qualifying employment offer in hand. No ITPE application can be filed without one. We work to match candidates with a qualifying employer through our partner network, though placement timing depends on current openings and is not guaranteed.

Years 1–4

ITPE License — Provisional Period

Practice at an NC-licensed hospital (anywhere in NC) or a qualifying rural medical practice. No phase change or second application required. A single license covers the full four-year period, regardless of placement type.

Year 5+

Full Unrestricted License

After completing the four-year period in good standing, you become eligible to apply for a full, unrestricted NC medical license with no geographic restrictions, and, for those coming from a rural placement, no continuing supervision requirement.

During the Four-Year Period

While holding your ITPE license:

  • you are employed and paid by your employer
  • required compliance obligations are maintained throughout the four-year period
  • changes in employment must be reported promptly to the NC Medical Board — for rural placements, this includes any change to your supervising physician
  • practice must remain within the scope and employer type authorized under the ITPE license

If employment ends for any reason, you must secure a new qualifying NC employer promptly. The NC Medical Board rules govern any gap period. Failure to maintain qualifying employment may result in license termination.

What PPG Does and Does Not Do

We prepare your side of the required documentation and bring you into a pool of thoroughly screened candidates presented to our partner employers. Confirming any required supervision arrangement (for rural placements) or credentialing (for hospital placements), and managing employer-side compliance, is handled by your employer's HR team, not by PPG.

We do not:

  • practice medicine
  • provide clinical supervision
  • make licensing decisions
  • make employer hiring decisions
  • guarantee employer placement
  • verify or confirm the employer's supervision arrangement
  • manage ongoing compliance after your ITPE-licensed practice begins

All licensing decisions are made by the NC Medical Board. All hiring decisions are made by the employer.
